Step 1: Identify the Level of Support Needed

The first step is determining what type of in-home support is required. This may include assistance with daily living activities, mobility, companionship, or respite for family caregivers.

Step 2: Explore Eligibility and Program Options

Some individuals may qualify for state-supported waiver programs that help cover the cost of non-medical in-home care services. Others may choose private pay options.

  • Community Health Choices

  • Act 150 Attendant Care

  • OBRA Waiver Services

Eligibility is determined by state agencies. Our role is to help you understand what may apply to your situation.

Act 150 Attendant Care

Act 150 is designed for Pennsylvania residents ages 18 to 59 with physical disabilities who are financially ineligible for Medicaid but require assistance with daily living activities.​

​

This program requires:

  • Nursing Facility Clinically Eligible determination

  • Ability to direct your own care

  • Completion of a physician certification form

OBRA Waiver Program

The OBRA Waiver supports individuals ages 18 to 59 with severe developmental physical disabilities that began before age 22.​

​

Eligibility includes:

  • Pennsylvania residency

  • Functional assessment

  • Financial review through the County Assistance Office

  • Physician certification documentation

Private Pay In-Home Care

If you do not qualify for waiver programs or prefer not to apply, HealedAtHome also offers private pay non-medical home care services.

We provide transparent pricing, structured scheduling, and clear expectations so families can make informed decisions.

Community Health Choices Program (Coming Soon)

Community Health Choices supports individuals age 21 and older who require nursing facility level of care and meet income and asset guidelines.

​

General steps include:

  • Applying through the COMPASS system

  • Contacting the Independent Enrollment Broker

  • Completing a functional assessment

  • Submitting required documentation

  • Completing the MA 570 Physician Certification Form

​

​Our team can help you understand this process and prepare necessary documentation.

LIFE Program (Coming Soon)

LIFE provides services and supports to help Pennsylvanians aged 55 and up live independently in their local community.

​

To be eligible for LIFE, you must:

  • Be age 55 or older

  • Meet the level of care needs for a skilled nursing facility or a special rehabilitation facility

  • Meet the financial requirements as determined by your local County Assistance Office or be able to privately pay

  • Reside in an area served by a LIFE provider

  • Be able to be safely served in the community as determined by a LIFE provider

Step 3: Gather Required Documentation

Most waiver programs require documentation to verify eligibility.

Common documentation may include:

  • Birth certificate

  • Social Security card

  • Proof of income

  • State ID or driver’s license

  • Bank statements

  • Proof of assets

  • Physician Certification Form MA 570
